Service Fees

Initial Diagnostic Evaluation 75 minutes $300.  Individual Psychotherapy Session 55 minutes $210

Insurances Accepted

Sugar River Psychotherapy is an in-network provider with Quartz and Dean insurance companies. Self-payment is also accepted.

Payment

We accept all major credit cards and health savings accounts for payment.

Policy for Out-of-Network Insurance

We are committed to supporting clients who wish to seek reimbursement from their out-of-network insurance companies. Clients are responsible for payment at the time services are rendered. Upon request, we will provide a monthly superbill—an itemized statement of services rendered with required insurance documentation. Clients are responsible for submitting the superbill to their insurance company and managing any follow-up required for reimbursement. Please note that reimbursement is not guaranteed and depends on your individual insurance plan. To request a superbill, contact Laura at 608-247-5968 or laura@sugarriverpsychotherapy.com. Cancellation Policy

The hourly rate will be charged for cancellations made without 24-hour notice or for missed appointments.

Good Faith Estimate

Under The No Surprises Act of 2022, you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. A Good Faith Estimate is for your awareness only. It does NOT involve you needing to make any type of commitment to the length or frequency of therapy sessions. The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
